Notice: Reentry Services for Incarcerated Juveniles Medicaid State Plan Amendment 25-0003 Public Notice is now available online.

Description: The Health Care Authority (HCA) intends to submit Medicaid State Plan Amendment (SPA) 25-0003 to provide targeted case management (TCM) services to eligible incarcerated juveniles. TCM includes a comprehensive assessment and periodic reassessment of an incarcerated individual in gaining access to needed medical, educational, social, or other services.
