Coverage will end on the last day of the month in which the SEBB Program receives your written notice.
If your written notice is received on the first day of the month, coverage will end on the last day of the previous month.
To cancel your supplemental life and/or accidental death and dismemberment (AD&D) insurance through MetLife submit MetLife’s cancellation of supplemental life and AD&D insurance form to:
