Kaiser Permanente WA Options Summit PPO 1

Summit PPO plans have 3 tiers of coverage for different networks of providers.

You receive the best value when you choose high-quality care from Kaiser Permanente providers. When choice is most important, you have access to more than one million in-network health professionals anywhere in the United States. You’ll have lower copays for office visits and most prescriptions if you use Kaiser Permanente Washington providers and pharmacies, and our community of specialists and hospitals systems.

Annual costs

  • Medical deductible: $1,250 per person / $2,500 per family for Tier 1 and Tier 2 providers; $3,750 per person / $7,500 per family for Tier 3 providers. (Deductible does not apply to preventive care, telehealth or prescriptions.)
  • Out-of-pocket limit: $4,500 per person / $9,000 per family

Key features

  • Acupuncture, chiropractic and massage therapy each have 24 visits per year
  • Rehabilitation has 60 combined visits per year
  • Unlimited naturopath visits
  • $300 plan allowance per year for orthotics
  • One hearing aid per ear, covered up to $3,000 every 36 consecutive months
  • No referral for most specialty care, and if a specialist procedure requires preauthorization, your physician will work with Kaiser Permanente. (Specialty care cost shares may apply.)

Available counties

In most cases, you must live or work in the medical plan's service area to join the plan. All school employees are offered a selection of plans based on their county of residence or the county where their employment location is based. Some school employees may have more plan options if they work in a district that crosses county lines.

You can choose this plan if you live or work in these counties:

  • Pierce
  • Snohomish
  • Spokane
  • Thurston

If you move out of your plan's service area or change jobs to a different employment location, and your plan is no longer available, you must select a new plan. If you do not, the SEBB Program will enroll you in one. You must report your new address and any request to change your medical plan no later than 60 days after your move.
