Description: The Health Care Authority (HCA) intends to submit a rate methodology for housing transition navigation services provided under the Health-Related Social Needs Program in the Medicaid Transformation Project, which is the state’s renewed Section 1115 Medicaid demonstration waiver. These services aim to remove barriers to affordable housing, including transition costs and housing deposits to assist with identifying, coordinating, securing, or funding one-time services and modifications necessary to help a person establish a basic household. The navigation services also provide pre-tenancy and tenancy sustaining services to assist with short- and long-term housing goals and remove barriers to stable housing.
